NOKOMIS HEALTHY SENIORS PROGRAM

Nokomis Healthy Seniors was established to enhance the lives of our community’s seniors by helping them to continue to live at home. We achieve this by offering affordable health services and coordinating volunteer assistance and community resources to serve seniors in our community.

Started in 1994 by seniors of the Nokomis community, Nokomis Healthy Seniors is designed to provide high quality, affordable services for seniors. Our services are more vital than ever due to rising health and home care costs and an increasing senior population.

Nokomis Healthy Seniors’ services have evolved to meet the needs of today’s seniors including independent living, affordable health services, and community participation. We help neighborhood seniors to continue living safely and independently in their homes by:

• Providing information and referral to community services
• Assisting in the coordination of services
• Recruiting and coordinating volunteers to provide services
• Providing nursing case management/health care coordination
