Horizon Health does amazing things for its community. A true support system to its community, Horizon Health provides services to the members of the area through direct care and volunteer service coordination.

Donate to Horizon Health through GiveMN.org on November 17th and your contribution will be matched with funds from the Minneapolis Foundation. Contribute between 8 am Nov. 17 and 8 am Nov. 18 on GiveMN.org and your gift will be amplified by sharing in a $500,000 match from The Saint Paul Foundation, Minneapolis Foundation and Bush Foundation. In addition, no transaction fees will be charged, so 100% comes back to us. In these economic times - it is great way to stretch your contribution dollars and support Horizon Health.

$25 This donation supports the coordination of volunteers that provide rides to seniors in the community that can no longer drive to appointments or to get groceries and home necessities.
$50 This donation may go to support respite care and caregiver support services for a member of our community that is supporting a aging or ill family member or friend.
$100 A contribution of $100.00 makes transportation services possible for groups of seniors and developmentally disabled community members to participate in cultural and community events.
$1,000 Provides for the construction of a wheelchair ramp, allowing a person to continue to live independently
