Annually we work with 150 community (community meaning Como Park and Falcon Heights communities) seniors and their caregivers. The Como Park Falcon Heights Living at Home Blcok Nurse Program is an organization that promotes wellness among neighborhood seniors by mobilizing professionals and volunteers to provide long-term, affordable home care and support services. By receiving in-home social support and health care, elders are enabled to stay in their homes preserving their quality of life and retaining their community networks, while at the same time avoiding expensive nursing home costs. We also focus on supporting caregivers so they are well informed and prepared to be healthy allies for their loved ones. Our programs emphasize health promotion, early intervention, management of chronic disease, and support and respite to caregivers. Our outreach is tailored to individual needs designed to help seniors in ways that support choice, dignity and independence.
