Horizon Health began the journey of providing services to aging adults and their families in 1991. We have evolved from an organization with 2 employees serving a handful of people to employing over 230 people serving 300 people on a daily basis. As you get to know us and our organization, you will notice that our services can meet a variety of needs from volunteer opportunities for people of all ages to in-home support services through our homecare department, independent senior living to assisted living services for frail aging adults and people with dementia or Alzheimers and the list goes on. We also provide services to those who are disabled in our community. We are here to help you or your loved one on your journey. Whatever path you are on Horizon Health is ready to be your guide.

Horizon Health does so much to help the community to be stronger and provide all people with the ability to live in a safe environment (including their own homes) throughout their lives. Great work and a great organization!

$ 100 A contribution of $100.00 makes transportation services possible for groups of seniors and developmentally disabled community members to participate in cultural and community events.
$ 250 This donation may go to support several of our community education programs, such as our Dine & Discover Series that provides seniors and caregivers with valuable information on how to remain healthy and thrive in their communities.
$ 500 This donation may go to support our valuable community advocacy programs that ensure the needs of the seniors and developmentally disabled in our community are known and their voices heard as programs are constructed and laws passed.
